It shows Frida's two different personalities. When Frida was first interviewed about it, she said it came from an imaginary childhood friend, but later revealed that it drew its inspiration from her loneliness from her divorce. On the modern Frida you can see that her dress is cut open and she is bleeding, while her traditional self stays fine. I'd say that this shows a disconnect from the two selves she shows the world.
