    I guess that both works of art are somewhat about agony.

    The Death of Ivan Ilyich is about a man who is living a meaningless life, which is similar to agony. On the other had, The Scream is about the pain of modern life, as it is an expression of agony which is a consequence of the fast pace of modernism.
