Strabismus can be detected by observing the position of corneal light reflexes and performing occlusion tests to identify tropias and phoria. This system is known as the Hirschberg test, it is done at 33 cm. of the patient, the fixation light and the deviation of the corneal light reflex from the center of the pupil in the non-fixated or rotated eye is evaluated. Each millimeter of decentration equals 7 degrees of ocular deviation. If we have a deviation inward of the light reflection of 4 mm. it would correspond approximately to an exotropy of 28 degrees, whereas if we found it to the outside, it would be an endotropia with the same value of degrees.
