A nurse obtains a health history from a mother who son is taking methylphenidate. she reports that he is having hallucinations. what would be the next action for the nurse to do?

    The best course of action in the given situation above is that the nurse should advice the mother or the patient if he or she can do so, to report the manifestation that the patient is experiencing when taken the drug and by that, the nurse should advice the patient to discontinue of taking the drug because the symptoms could have been taken an effect because of the drug.
