When auscultating a client's lungs, the nurse hears a sound like velcro being pulled apart over the client's right middle lobe. how should the nurse document this finding?

    She should indicate in which part is heard it, in this case "apparently in the right middle lobe of the client", also should report what time of bheath is heard it, in addition to comparing the sound using the stethoscope and without.

    Once written in the report, she should inform the doctor about the case to discuss the following procedure or treatments.
