The client states she has pain to her left forearm four (4) hours after the start of the infusion. upon inspection and palpation of her forearm, the nurse notes redness and swelling at the iv site and the forearm is warm to touch. what is the initial action the nurse should take?

The nurse should check whether the IV line is clogged. Four hours is too fast for infection so the swelling probably is caused by a blockage of the fluid. The nurse can try to correct the position of IV line or flush NaCl to the infusion to remove the clot.

If the IV line clot and cannot be fixed, the IV line should be removed and changed to another site.
