The answer should be: Acute Pyelonephritis

Pain in the urinary tract can be caused by infection or obstruction. Fever in this case indicates there is an infection. The urinary tract nervous system will cause referred pain where lower urinary tract pain will be felt in the groin and upper urinary tract pain will be felt on the back.
