    The irony is that they are walking to a farm, where they are expecting to be well taken care of, when actually that's where they will be killed for a resource (their meat). The cartoonist's intent is probably to denounce the fact that in the U. S., healthcare is so expensive for the more vulnerable people (like the elderly) that they feel like when they enter a hospital, they have no choice but to surrender their resources for medical care.

    It could also be a metaphor: the farmers care very little about their turkeys' healthcare because it costs too much considering they will be slaughtered anyway, the same way the government does not prioritize spending on health coverage for its citizens.
