The answer is Physical damage caused psychological damage.

The author used this section: "We left Vietnam peculiar creatures, with young shoulders that bore rather old heads" to express not only the fatigue to be in the war but also the anguish to have the death too near without a hope of life. He made a comparison between a young mind that feels alive and it believes it could be eternal, and an old-middle mind, that feels afraid to die in any moment and to delete forever his existence. He explained in that time they were young but lived so hard experiences that made them they felt on the verge of death.
