Annie's flashbacks tell us that she used to be in an asylum with her brother. They used to play with rats in a "death house". This affects the way she deals with Helen because her mother, Kate, didn't want Helen to be sent to an asylum. Annie knew how bad asylums were, and how isolating. She understood the fears Helen's mother had.
