This format is the most grammatically correct. It has the clearest and most succinct wording. Also, the end citation is correct. The footnote should end with the source that's being referred to, not begin with it. The correct punctuation is a comma follow by "see", the author's last name (or title, if there is no author) and the page numbers. Options A and B put the reference information first. Option D includes an extra comma that is not needed.
