existed only in a few colonies

    Explanation:

The Seperation of church and state constitutes that freedom of religion is protected by the First Amendment of the United States constitution, that prohibits laws establishing a national religion or impeding the free exercise of religion for the citizens.

The Amendments enforces the Seperation of church and state. The first Amendment was adopted on December 15, 1791 and establishing a Seperation of church and state that prohibited the federal government from making law respecting establishment of a religion and also prohibit government from interference with a person's religion practices and beliefs.

But this however, existed only in few colonies, several other colonies were intolerance and interfered with people's personal religion. The Spanish that occupied Florida for instance massacred and hung the Huguenots, a group of French Protestants that occupied a colony at Fort Caroline near modern day Jacksville, Florida. Also, Puritans and Pilgrims that arrived New England in early 1600s after suffering and witnessing religious persecutions in England, the Puritans of Massachusetts Bay colony did not tolerate opposing religious views and several Catholics, Quakers and other non-puritans were banned from the colony. This also applies in several other colonies and only Rhode island which was found by Roge Williams, a Puritan dissident banned from Massachusetts and move South becomes the first colony with no established church and first to grant religious freedom to everyone.
