    Our quadratic polynomial function is:

    f (x) = 4x^2 + 2x + 5

    our linear binomial form is:

    x - a = - 2

    hence:

    a = x + 2

    and so we have:

    f (x) = 4x^2 + 2x + 5

    f (a) = 4 (x + 2) ^2 + 2 (x + 2) + 5

    = 4 (x^2 + 4x + 4) + 2x + 4 + 5

    f (a) = 4x^2 + 18x + 25
