    I don't know what general 5' form means, but I'm guessing that means slope-intercept form, the easiest form of a linear equation. So we can use the equation for this is y = mx+b. M being slope, and b is the y-intercept. So that means that we will get an answer of:

    y = 1x+5, you spelled 1 wrong so I just correct it.

    Answer: y = x+5
