It is D because 25 percent is a fraction or a decimal: 25/100.

Therefore A is correct because 25/100 in decimals is 0.25,

B is correct because 25/100 is 25 percent,

1/4 is the same as 25/100 as a fraction

And finally, D is wrong because 25 is not in fraction/decimal/percentage form

In this case, N is just a variable. Lets make N 100.

    A. 0.25*100=25 25 is 25% of 100 so this checks out.

    B. 25 (100) / 100=25 25 is 25% of 100 so this checks out too.

    C. 1/4 = 0.25 (100) = 25 25 is 25% of 100 so this checks out as well.

    D. 25 (100) = 2500 2500 is greater than 100 so its more than 100% of 100.

As to why that is, its because you're multiplying the variable by 25, but that wont get you 25% of the variable.
