    The most appropriate nursing diagnosis for a client/patient that has heart failure would be "ineffective tissue perfusion." A patient that is suffering from heart failure will experience pain, being fatigued more than regular, and activity intolerance. The patient may also begin to have low blood pressure, confusion, and slow capillary refill. To be diagnosed there are several tests that must be taken by a licensed professional.
