Root cause analysis is used to find the root cause and to improve it. During this a series of questions are posed to find out the cause of the issue. Incident investigation and problem solving are some of the root cause examination. Root cause analysis is connected to three basic questions; what is the problem and why did it happen, what can be done to prevent it from happening again.
