    Answer:4) action stage

    Explanation:

What happens during the action stage?

After a preparation stage (which is a stage when someone is getting ready to actual start an action to change) comes the action stage.

This is the stage in which someone takes the necessary actions that will modify the behaviour that they need to change and this stage occurs in the six month period.

This isn't done secretly, it is an open process.

An action may be prescribed by a doctor or a psychologist for example if someone is an addict they may get a recommendation to attend a rehab in order to recover from the addiction or they may need to visit the Alcoholics Anonymous that is taking action towards changing the unwanted behaviour.
